Interpreting and Using Post-school Outcome Data for Program Planning

In this session, we will explore how to interpret post-school outcome data for your district/ESD. As a group, we will brainstorm the meaning of the data and what questions still need to be asked. Finally, we will discuss the ways that the data can guide program planning at the district, school, or student level.

High Expectations! Collaborative Supports for Secondary Students with Disabilities

This session provides an overview of collaborative initiatives in support of improved outcomes for secondary students with disabilities, including current best practices and resources in support of transition planning. The discussion will include a review of current transition data and outcomes, strategies for aligning High School and Beyond plans with transition planning in Individualized Education Programs (IEPs), and evidence-based resources to support transition planning.
Teaming for Transition: Preparing Students for Postsecondary Life

Planning for students’ transition needs requires a coordinated approach between IEP team members. Our district has embarked on a journey to make this a smooth process that addresses students’ transition needs at the high school level and through specific programming for some students ages 18-21. Coordination with community partners, connections with outside agencies, and careful scheduling of instruction are all aspects of our teams’ work to build transition supports for all students.
